Merge accepted core-only input receipts and presence ceiling

Replace the superseded proposal-only USER_INPUT path with the accepted core-only receipt implementation. Remove InputPathProposal, its inert authorization/dispatch path, four obsolete cells, and REQ-USER-INPUT-PATH-PROPOSAL / REQ-USER-INPUT-PATH-AUTHORIZATION stanzas and evidence. No proposal authorization behavior remains. Retain quoted_path_candidates as the shared scanner used by current receipts, and preserve independent attach admission/client-policy behavior. Restore the missing array header before REQ-INPUT-PROVENANCE-ACCEPTANCE-REPORT inherited from a23fca1f. Includes accepted #299 2.5-second ceiling.
