docs(infra): register IR-146 — the Quality audit reports 87% of the registry

`traceable-reqs lint` emits 1496 [must] findings over 825 of 949 requirements
(length 819, contains-and 674, tbd-todo 3), measured with checker 0.4.1 at
6c95d691. A finding class that fires on 87% of the corpus is a calibration
object, not a per-lane defect — but the CI step is `|| true`, so its only
reader is whoever opens the log, and a gater reading a touched id's two
findings mistakes house style for lane debt.

Recorded with the mistake attributed: I routed two such findings to the H3
lane as carry-forward before measuring the population, and withdrew it the
same day.
